• CSD Supplier to provide LATEST CSD summary report

• Commissioner Of Oaths Completed, signed and stamped by the Supplier

• SBD1 Form Complete and sign the document by Supplier- in case a person completing the form is not a director, a letter from your company must be provided authorizing you to sign on behalf of your company as per

instruction on the form under CAPACITY UNDER WHICH THIS BID IS SIGNED (Attach proof of authority to sign this bid; e.g. resolution of directors, etc.)

• Annexures Complete and sign the documents by Supplier (Annexures C & E) – If Applicable

• CORP 39 Documents Supplier to forward
