• Fully completed and signed SBD 1 must be attached.
• Fully completed signed SBD 4 must be attached. A false declaration will result in
disqualification. Paragraph 2.3 disclosure must align with subsection 2.3.1 and
directors’ information on the Central Supplier Database (CSD).
• Fully completed and signed SBD 6.1 must be attached.
• The pricing schedule must be fully completed.
• A valid Letter of Good Standing for the Compensation for Occupational Injuries and
Diseases Act (COIDA) in Gardening services/Landscape services issued by the
Department of Labour.
